Medicare Rules for Incident-to Billing and Shared Services

With the 2022 change to shared services, it’s a good time to review the unchanged incident-to rules and the updated shared services rules. Incident-to services are services performed in a non-facility setting by clinical staff or non-physician practitioners. Shared services are E/M services jointly performed by a physician and non-physician practitioner in a facility setting. Understanding the rules for these is both a revenue and a compliance imperative.

After the session, participants will be able to:

  • Identify the financial implications related to incident-to billing and shared services
  • List the types of services that can be billed incident-to
  • Describe the documentation needed for shared services

Join us for this live, interactive webinar to audit three E/M notes, one primary care note, one medical specialty note, one surgical specialty note. Using the sample notes, the webinar will review the three elements of medical decision making. There will be many poll questions, so that participants can communicate their assessment of the components of each note and compare their assessment with their peers.

  • Name three problems commonly seen in their practice and determine if those problems are minimal, low, moderate or high

Coding critical care services 2022

CMS made significant changes to critical care coding effective in 2022. Not only did CMS adopt CPT prefatory language and rules, they did a 180 degree turn on allowing non-physician practitioners (NPPs) and physicians of the same specialty, in the same group to perform critical care as a split/shared visit. CMS developed a HCPCS modifier for shared services, that must be applied to the claim, and the claim must be submitted under the provider number of the practitioner who performed the substantive portion of the service. CMS developed a HCPCS modifier for unrelated critical care in the post op period. This webinar will describe when that modifier should be used.

After the webinar, participants will be able to:

  • Describe what modifier to use for shared services
  • Know how CMS determines the substantive portion of the service for critical care
  • Assess when a separate E/M service may be reported on the same day with critical care
